1 BLS & AED: Slajdy dodatkowe

Wentylacja workiem samorozprężalnym z maską Generalne zasady użycia masek twarzowych Użycie tlenu Zadławienie Tonięcie Resuscytacja w dwóch ratowników It is not assumed that all courses will wish to teach all the topics in this supplementary material, though of course you may. It is anticipated that topics from this list will be chosen as required.

4 Ocena tętna Nie zalecana dla ratowników niezawodowych
Ułóż palec wskazujący i środkowy w zagłębieniu na szyi zawsze tylko po jednej stronie Jednocześnie oceniaj oddech i tętno Odchyl głowę Nie dłużej niż 10 sekund Can also be difficult for trained rescuers Check breathing and carotid pulse at same time, before starting chest compressions. Maintain chin lift Use the index and middle fingers on one side only of the neck Place the tips of your fingers in the gap that can be felt between the thyroid cartilage and the sternomastoid muscle, at the level of the thyroid. Check for no more than 10 seconds.

If pulse definitely present, when re-checking for signs of life after 1 minute: If pulse still definitely present, continue with rescue breaths for another minute, and then re-check If pulse now absent or unsure, start chest compressions, and do not re-check for carotid pulse or for signs of life

6 OGÓLNE ZASADY UŻYCIA MASKI TWARZOWEJ
Odchyl głowę do tyłu Zapewnij dobrą szczelność pomiędzy maską a ustami i nosem pacjenta Wykonaj wentylację w trakcie 1 sekundy Klatka piersiowa powinna się unieść i opaść jak przy normalnym oddechu Nie wykonuj hiperwentylacji Wykonuj 30 uciśnięć klatki piersiowej naprzemiennie z 2 wentylacjami

7 UŻYCIE TLENU Jeżeli dostępny użyj tlen w trakcie prowadzenia resuscytacji Możesz go podawać poprzez worek samorozprężalny lub maskę kieszonkową Podaj go tak szybko jak to możliwe w możliwie największej ilości: Przepływ l/min Oxygen can be given during CPR: If giving oxygen using a pocket resuscitation mask: Many will have specific connections If not, place oxygen tubing underneath one side of the mask, and create a good seal between the mask, tubing and victim’s mouth. Use the pocket resuscitation mask in the usual way to deliver rescue breaths through the one way valve Deliver rescue breaths over 1 second. Give enough for the chest to rise and fall. Avoid rapid or forceful breaths.

Oxygen can be a hazard when using AED. To minimise the risks: Firmly sticking down pads to minimise the risk of generating sparks Remove oxygen source > 1 metre from the victim’s chest during shocks.

Niedrożność łagodna Zachęcaj do kaszlu Ściśle kontroluj stan poszkodowanego Poszkodowani z ciężką niedrożnością dróg oddechowych wymagają natychmiastowej pomocy: Uderzenia w okolicę międzyłopatkową Uciśnięcia nadbrzusza Poszkodowany nieprzytomny Wezwij pogotowie Rozpocznij RKO Choking commonly occurs due to airway obstruction with food. Treatment of choking is required urgently Mild airway obstruction These people may still be able to speak, cough, move or nod. You should encourage these people to continue coughing but do nothing else. It is important to continue to monitor these victims to ensure that they do not deteriorate Severe obstruction: Those who are having increasing difficulty in breathing, who are unable to speak or are unconscious have severe airway obstruction and require further help.

10 ZADŁAWIENIE: ALGORYTM
Ciężka niedrożność Łagodna niedrożność Poszkodowany nieprzytomny Poszkodowany przytomny Zachęcaj do kaszlu Ściśle kontroluj stan poszkodowanego Wezwij pogotowie Rozpocznij uciskanie kl. piersiowej Kontynuuj uciskanie kl. piersiowej i oddechy ratownicze 5 uderzeń między łopatki 5 uciśnięć nadbrzusza Back blows: Stand to the side and slightly behind the victim. Support the chest with one hand and lean the victim well forwards Give up to five sharp blows between the shoulder blades with the heel of your other hand Check to see if each back blow has relieved the airway obstruction. Give further back blows only if the obstruction is not relieved Abdominal Thrusts: Stand behind the victim and put both arms round the upper part of his abdomen. Lean the victim forwards Clench your fist and place it between the umbilicus and xiphisternum. Grasp this hand with your other hand and pull sharply inwards and upwards If the obstruction is still not relieved, continue alternating five back blows with five abdominal thrusts. Unconscious victim: Support them into a supine position on the ground Call the emergency services Start chest compressions. Continue by combining 30 chest compressions with 2 rescue breaths

In a drowning victim the duration of hypoxia is the critical factor in determining the victim’s outcome. Therefore, a modification to the normal BLS algorithm is required in drowning victims to allow earlier delivery of rescue breathing Casualties who are actually in water should be removed as soon as possible. Avoid going into the water yourself if at all possible. Talk to the victim, use rescue aids or a boat or similar water vehicles. Rescue breaths in water are difficult to perform but can be done if you have been specifically trained how to do this. Chest compressions should not be attempted in the water. An AED can be used out of water or on a rescue boat. Dry the victim’s chest before attaching AED pads to ensure adherence. 5 oddechów ratowniczych 30 uciśnięć 2 oddechy ratownicze Jeśli jesteś sam zadzwoń 112 po 1 min.

13 RKO PROWADZONE PRZEZ 2 RATOWNIKÓW
30 uciśnięć : 2 oddechy ratownicze Pierwszy ratownik wykonuje oddechy ratownicze Drugi wykonuje uciśnięcia klatki piersiowej Użycie AED: Pierwszy ratownik obsługuje AED i nakleja elektrody Drugi wykonuje RKO Przerwy w RKO tylko na analizę rytmu i wykonanie defibrylacji The rescuer delivering rescue breaths should keep the airway open when the other rescuer is delivering chest compressions. The ratio of 30 chest compressions to 2 rescue breaths remains the same in adult BLS for two rescuers Because of rescuer fatigue chest compressions are performed less effectively after a short period of time. The rescuer performing chest compressions should be changed – with minimal delay – every two minutes to ensure that high quality chest compressions are performed throughout the resuscitation effort. When using an AED, one rescuer should operate the AED and apply the electrodes while the other rescuer continues with chest compressions and rescue breaths. Interruption to CPR should only occur during rhythm analysis by the AED and when delivering a shock. A shock should be delivered as soon as it is advised, ensuring that no rescuer is in contact with the victim. Kontynuuj uciskanie klatki piersiowej w trakcie naklejania elektrod AED
