Key message: IBM is bringing a set of solutions into the marketplace to enable holistic and individualized care that is being requested by the citizens of the world, with a focus on optimized outcomes. This is our opportunity together to address quality, costs and outcomes, and drive individual wellness and community vitality. Three solution layers: Foundation – the kinds of solutions that are targeted at recognizing opportunities for intervention, based on the coalescing of all of the data – and enabling evidence-based and standardized care planning -- Know where this data comes together; data that sits in electronic medical records, individual patient profiles on someone’s laptop, data that sits in someone’s office whether a physician’s office or social worker’s office; or data that sits in multiple external sources and is difficult to access. -- The foundation set of solutions pulls all of that data together to begin to build this holistic view, and provide a view of the individual’s issues and concerns – from a lifestyle, social and clinical perspective. 2) Analytics and Cognitive Computing – are the next set of solutions. Now that I have all of this data, what do I do with it? What can I better understand / how can I learn from it? How do I ensure the processes that are leveraging this data are doing so on behalf of improved outcomes? How do I ensure that when I am making a decision, I am making an informed decision based on the right kind of data. How do I learn from the decisions that I make? -- The powerful thing about cognitive computing is that it forces us to constantly learn. The more data we have, the more we know, the more we analyze, the more knowledge and insight we have. It’s a continuous loop, and every cycle through yields more understanding and action, and opportunity to improve outcomes, and cost. 3) Coordination is the 3rd layer. This is about our ability to engage the individual and the family, and all of the stakeholders – and drive collaboration across communities of care, at the right time. Ensuring that we have all of the right members engaged who impact care, and drive individualized care plans – and optimal outcomes. 3 3 3
